Qualifications: • Must be a graduate of an approved school of professional
• Must be currently licensed in the State of Florida as a
registered nurse.
• Must have one (1) year’s experience in an acute care setting
and/or home health care.

Responsible To: Agency Director of Nursing, Administrator
Responsibilities: • Can act as a Case Manager, involving nursing or both
nursing and therapy care.
• Assess patient and family to determine type, appropriateness
and adequacy of services.
• Can make initial assessment of the patient’s needs and
development of patient care plan
• Establishes and periodically reviews plan of care.
• Implements medical treatment orders, referrals, follow-up,
provision and supervision of nursing care.
• Documents all activities and findings
• Responsible for the clinical record for each patient receiving
• Assure that progress reports are made to the physician when
the patient’s condition changes or there are deviations from
the plan of treatment.
• Can be responsible for supervisory visits to the patient’s
home if receiving care from a licensed practical nurse or
home health aide.
• May assign selected portions of patient care to the licensed
practical nurse or home health aide under supervision.
• Responsible for following the physician’s orders and
reporting to the physician the patient’s condition and
• Informs other personnel of changes in the patient’s condition
and needs.
• Makes the initial evaluation visit, initiates the plan of care
and necessary revisions.
• Makes scheduled visits and regularly re-evaluates the

patient’s nursing needs.
• Provides those services requiring substantial specialized
nursing skills, initiating appropriate preventive and
rehabilitative nursing procedures.
• Prepare clinical and progress notes relating to all aspects of
patient care.
• Coordinates services including all disciplines.
• Assumes self-responsibility of all direct care activities and
delegation of activities administered as indicated by the
Nurse Practice Act and Home Health Care policies and
• Provides for and gives direct and indirect care according to
the plan of care while assessing the total patient care
measures based on the individual patient needs.
• Observes, monitors and records the patient’s responses to
care and treatment.
• Teaches, supervises and consults with the patient and family
members in methods of meeting the nursing care needs and
other related problems of the patient at home.
• Initiates emergency measures in life-threatening or unsafe
conditions until the physician can be made available.
• Prepares and receives a report which will provide continuity
of care for all patients.
• Maintain the confidentiality of the patient’s clinical record as
mandated by the Health Information Portability and
Accountability Act (HIPAA).
• Prepares and submits progress notes.
• Coordinating services given by other health care providers,
documenting all activities and findings.
• Reports all changes in the patient’s condition to the
supervisor and physician.
• Counsels the patient and family in meeting nursing and
related needs.
• Participates in in-service programs.
• Supervises and teaches other nursing personnel
